Office Visit

For your convenience you can download and complete the forms posted on this page and bring them to your first appointment with Dr. Stosel. A copy of the Patient Privacy Practices will be available for your review when you arrive for your appointment.

If you have other relevant materials that you would like to share with Dr. Stosel, please bring copies to your appointment.

Dr. Stosel contracts with insurance carriers Blue Shield, Atena, Cigna PPO’s as well as Medicare. Otherwise she is an out of network provider. Please bring your insurance card to your appointment. Be sure to notify our office of any changes in insurance.

Payment is required at the time of each visit. For your convenience, Dr. Stosel excepts Visa, MasterCard and Discover card, along with personal checks and cash.

Office Fees

Routine new patient visits are $303.00 for up to a one hour eval. Follow up visits are $153.00 for up to one half hour visits. Onsite injections and testing are extra.

Important

Your insurance is the contract between you and the insurance company. You are responsible for your bills regardless of what insurance pays. Please have all your insurance information available to us and if and when your insurance coverage changes in any way. We accept major credit cards, checks or cash.

Our primary purpose is to provide you with the highest quality endocrine care with courtesy, respect and understanding. Our success can be measured only by your satisfaction with the health care you receive. We encourage your comments as to how we may better serve your needs.
